If we need further information on your pet’s condition, we may call you – please note that we will be calling from 1(408) 262-2133 and it will show up on your phone log as ‘HUMANESOCIETYSV’.

Step 1

Select the appointment type that aligns with the primary issue your pet is experiencing:

  • Tier 1 (Eye Discharge, Cold Symptoms, Urinary Problems, Limping)
  • Tier 2 (Minor Wounds, Skin Problems)
  • Tier 3 (Vomiting, Diarrhea, Ear Problems)

Step 2

Please complete the entire form, including items marked “optional.”

  • Be sure to include your name, email, phone number, zip code, your pet’s name, and your pet’s breed. Requests that are incomplete or missing this information may not be accepted.
  • Under “Select provider” select “Essential Care Clinic Appointment” or “Essential Care Clinic Appointment 2.”

Please note: If your pet is experiencing a critical or life-threatening emergency (such as difficulty breathing, actively seizuring, and/or collapsing), contact the nearest emergency veterinary clinic immediately.

We don’t want pets to go without services.  Please ask about options during your appointment.
